Tom had been living in a group home for many years, but found the environment challenging and was struggling to achieve his goals. He had always dreamed of living in his own home, with the freedom to make his own choices and live life on his terms.

Tom’s NDIS plan included funding for Supported Independent Living (SIL), which would allow him to live in his own home with support from carers as needed. He engaged a provider who specialised in NDIS SIL accommodation and began the process of finding the right home and support team.

After a thorough assessment process, Tom was matched with a team of carers who understood his needs and goals, and who were committed to supporting him to live as independently as possible. Together, they worked to find a suitable home that met Tom’s needs and preferences, including accessibility and proximity to amenities.

With the support of his SIL provider and carers, Tom moved into his new home and began to settle into his new routine. He enjoyed the freedom of being able to make his own decisions, and felt empowered by the control he had over his own life.

Today, Tom is living a more independent and fulfilling life thanks to the support of his SIL provider and carers. He has achieved his goal of living in his own home, and is looking forward to exploring new opportunities and experiences with the ongoing support of his team.
